What are Sunflower Haven's fees for service?Our hourly rates range from $23 to $27/hour when you request > 4 hours of care in a day. We encourage you to contact our office for special rates offered for live-in care, 24-hour shifts, respite care and 1-hour drop-in services. We develop a Personalized Price Package and Service Plan that works for your budget.

How often are Client Care Plans re-evaluated/updated?Sunflower Haven Clients receive a review of service assessment every 3 months or sooner if needed. During the review of client services, one of our Agency Supervisors will carefully review your current care plan and also discuss your overall satisfaction with the care being provided. Clients are encouraged to call us immediately with any pertinent information or care plan modification requests. In most instances, a new home visit is not required to make minor adjustments to the current care plan.

What languages, other than English, do your employees speak?"We will make every attempt to match our clients with a provider who speaks their preferred language. If we are initially unable to find a provider who meets the preferences of the client, the client is not obligated to remain under contract with the Sunflower Haven Agency.

What services are provided by the Sunflower Haven Home Care Agency?Sunflower Haven is a Private Duty Agency who provides Non-Medical assistance with Activities of Daily Living also known as ADL's. How are your caregivers managed/supervised when they are at a client’s home?Sunflower Haven providers are employees, NOT INDEPENDENT CONTRACTORS. We provide each caregiver with a detailed Care Plan accessible electronically and in paper form to ensure that the client's wishes are carried out as requested. Our providers have direct access to our agency supervisors 24 hours per day in case they have questions or concerns about their care plan. If constructive feedback is required from the client or family members, our providers are trained to accept it with patience and understanding with the overall goal of delivering exemplary care to our clients. Our Supervisors periodically perform unannounced visits during caregiver shifts to ensure that quality care is being provided to our clients.

What is an In-Home Assessment? What is the cost of an In-Home Assessment?An In-Home Assessment is a meeting between a Sunflower Haven Agency representative and the prospective client and their loved ones. The meeting usually takes place at the location considered "home" to the prospective client. The assessment takes approximately 45 minutes. During the assessment, the following are reviewed: Personalized Care Plan Home Safety Check List Review of Agency Policies and Contract Review of Client's Caregiver Preferences Review of Client's Rights and Responsibilities Individualized Budget Friendly Pricing Package Sunflower Haven provides a No Obligation In-Home Assessment FREE OF CHARGE to all prospective clients.

What are the minimum number of hours required to request services from Sunflower Haven?There are NO MINIMUM hours required. However, we do offer an hourly rate discount for clients who request > 4 hours per shift.

What are the minimum hours of training required for Sunflower Haven caregivers?Sunflower Haven care givers are required to undergo a minimum of 8 hours of training prior to their first 30 days of employment and 8 hours of additional training annually thereafter. Most of our providers have years of experience providing assistance with activities of daily living. For providers who are new to the field, we have a mentoring and shadowing program in addition to our training program which allows them to shadow personal care attendants and nursing assistants to learn and become familiar with the needs of each client they are assigned to.

What is the after hour or emergency contact procedure for Sunflower Haven?In the case of a medical emergency or if the safety of a client or provider are at risk, 911 should be called immediately. in the event you need to speak with a member of the Sunflower Haven staff urgently, please dial the main office line and press 0 (zero). You will be connected to the on-call manager or asked to leave a message and the on-call manager will return your call within 30 minutes.
